先日、あるキッカケで医療に関する情報管理の勉強をしました。
統計や確率の勉強がほとんどだったこともあり、
久しぶりの頭の体操になったという感じです。
現時点では仕事には活かせそうにないですが、
仕事と関係ない分野の勉強も悪くないです。
統計の勉強をしていて、また、先日痛ましいエレベータ事故があったことで
思い出したのですが、「ハインリッヒの法則」というものがあります。
１回の大惨事の裏には、29回の小事故があり、その背景には300回のヒヤリ・ハット
（事故には至らなかったものの、ヒヤリとしたハッとした事例）が存在する法則です。
これはハインリッヒ氏が労働災害を統計学的に調査し、導き出した法則です。
この統計法則は、社会では保険料率等で活用されています。
私にとっての教訓は、「ヒヤリハット」な事象こそ見過ごさずに反省し、
その原因を調べ、そのようなことが起こらないよう注意・対処していこう、
というものです。
医療分野で、ヒヤリ・ハットの例を引用してみましょう。
「ある医師（もしくは看護士）が間違った薬を患者に渡したが、
患者がいつもの薬と違うことに気づき、服用しなかった。」
「患者は間違った薬を渡されたため、その薬をそのまま服用したが、
それによって健康被害は起きなかった。患者からのクレームもなかった。」
などになります。
この種の”結果的に異常は無かったが、ミスはあった”というヒヤリハットは、
葬儀の現場においても山ほどあるでしょう。
そして、なかなか管理者の目や耳には入り難い情報です。
結果として異常が無かったら、ミスは報告されにくいものですからね。
しかし、そのヒヤリハットを細かく管理・チェック・対応しておくことこそ、
大事故（クレーム）に繋げないための予防薬です。
弊社の司会研修でも、研修内容を打ち合わせる段階で、
「ミスを減らすためには、どうしたらよいか？」
という相談を受けることがあります。
ミスを減らすことは、葬儀司会においても課題の一つです。
研修では、具体事例を挙げて対処法を挙げたり、
ミスを分類してポイントを説明したり、
講師（社長）の経験も踏まえながらミスを減らすコツを指導したりしています。
私個人の考えでは、日々「記録」を付けることがミスを減らすことに繋がると考えます。
施行報告書など、ある程度の形式（フォーマット）は必要でしょうが、
文章の書き方（文体）は堅苦しくなく、その人らしい記録で良いのです。
ポイントは、定期的に、継続的に記録を付けること。
定期的に継続的に付けるからこそ、ちょっとした異変にも気付きやすくなります。
また、継続的だからこそ、記録が無い日にも「記録が無い」という記録が残ります。
「何かあったのかな？」と思う訳です。
医療の情報管理の勉強から話が逸れましたが、
今日はそんなエッセイです。
